1. Donation Requests
Any organization or cause wishing to receive a charitable donation must submit a formal request to the Charitable Giving Committee. Use the form below to include:
-The organization name, contact person, phone number, email and mailing address.
- A description of the organization and its mission.
- A specific breakdown of how the funds will be used.
- Evidence of the organization's credibility and tax-exempt status
(if applicable).
-If a donation request is for a charitable event, the request must be submitted no less than thirty (30) days before the event date.
